Request for Note-Taker Assistance

This form only applies to students who have been formally approved to receive accommodations through the Office of Disability Services.

A note-taker is an accommodation approved through the Office of Disability Services for students who qualify for assistance in acquiring a supplementary copy of notes from in-classroom sessions. If you have been officially approved for assistance with note-taking and are seeking to request note-takers for your current classes, please fill out the following form. The Director will then contact you to set up the note-taking services for the specific classes that you specify.
